After you print your healthcare directive, carefully follow the included instructions. Your Directive will not be complete and effective until you have signed it, AND it is either witnessed by 2 people OR notarized.

After creating your healthcare directive, make sure to store it in a safe place, give a copy to your healthcare agent, and let your loved ones know how to access the document in an emergency.
